HEART SUCCESS

Heart Success is a program to help patients with heart failure avoid going to the hospital. Patients and their families are taught to recognize and treat symptoms early, thereby preventing hospitalization. Patients also learn how to take medications for maximum benefit and minimal side effects.

Services include:

• Care directed by your physician
• Assessment between physician office visits
• Individualized healthy eating plans
• Risk factor modification
• Medication education
• Education to help prevent and manage symptoms
