Social Work Services
Our Social Work Services Department seeks to enhance the quality of patient care, support the delivery of health care services and promote community outreach programs. We work to provide expert, timely, quality care close to home.
We provide clinical social work services and discharge planning for all our patients -- from infants to geriatric -- who receive care and treatment at MedCentral. Social workers participate in multidisciplinary discharge meetings and work closely with the patient care team to develop a discharge plan and provide continuity of care.
