MedCentral named to 2012 HomeCare Elite
MedCentral has been named to 2012 HomeCare Elite, a compilation of the top-performing home health agencies in the United States.
Now in its seventh year, the HomeCare Elite identifies the top 25 percent of Medicare-certified agencies and further highlights the top 100 and top 500 agencies overall. Winners are ranked by an analysis of publicly available performance measures in quality outcomes, process measure implementation, patient experience, quality improvement and financial performance.
MedCentral Home Care Director Marte Griffeth credits “quality initiatives and continuity of care practices,” with the department’s ability to rank as one of the HomeCare Elite.
“We utilize real-time benchmark reports to quickly adjust practices/processes and keep the staff informed as to their individual scorecards to improve individual as well as agency performance. Regarding ‘Continuity of Care’, MedCentral Home Care uses a ‘case management’ model where patients are assigned to the same registered nurse or physical therapist, so the patient primarily sees the same clinician and care is not piecemealed; patients receive the best care this way,” she said.
